Self Pay
If we are in-network with your insurance, we will file claims on your behalf for the payment of services rendered. Clients who wish to use insurance will be responsible for any deductible or copayment at the time of service. These amounts are determined by your insurance plan. Please note that most insurance providers only cover services deemed medically necessary, which means any diagnosis will be shared with your insurance for payment purposes. Additionally, there may be limitations on the services or diagnoses covered by your insurance. For any questions about your coverage, please contact your insurance provider directly.
If you are requesting services as a self-pay client, we will provide a Good Faith Estimate outlining the anticipated cost of your sessions based on your individual needs.
Cancellation Policy
If you do not attend your scheduled appointment, and you have not notified me at least 24 hours in advance, you will be required to pay a $65.00 fee for the session.
Payment
We accept cash, checks, and all major credit or debit cards. We kindly request that clients keep a card on file to cover any balances not paid by insurance or other means. Electronic payments are processed through Ivy Pay, a secure and encrypted platform that protects your personal and payment information.
Intake Paperwork
All intake forms will be sent through Simple Practice client portal once we have decided to schedule. These forms will need to be completed prior to confirming an intake appointment date and time.
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.
Under Section 2799B-6 of the Public Health Service Act, health care providers and facilities are required to give patients who don’t have insurance—or who are not using insurance—an estimate of expected charges for medical services, including psychotherapy.
You can ask for a Good Faith Estimate when you schedule a service—or at any time—from any of your providers who are not billing your insurance for your services.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute it.
Make sure to save a copy or photo of your Good Faith Estimate for your records.
